This notice describes how health information about you may be used and disclosed, how you can obtain access to this information, and your rights regarding your health information. Please review it carefully.
Your Rights
When it comes to your health information, you have certain rights. This section explains your rights and some of our responsibilities.
Get an Electronic or Paper Copy of Your Health Record
You may ask to see or receive an electronic or paper copy of your medical record and other health information we maintain about you.
We will provide a copy or summary of your health information as required by law. We may charge a reasonable, cost-based fee when permitted by law.
Ask Us to Correct Your Health Record
You may ask us to correct health information about you that you believe is incorrect or incomplete.
We may deny your request in certain circumstances, but if we do, we will explain the reason in writing as required by law.
Request Confidential Communications
You may ask us to contact you in a specific way, such as at a particular telephone number, or to send correspondence to a different address.
We will accommodate reasonable requests.
Ask Us to Limit What We Use or Share
You may ask us not to use or disclose certain health information for treatment, payment, or healthcare operations.
We generally are not required to agree to these requests.
However, if you pay for a healthcare service or item completely out-of-pocket, you may ask us not to disclose information about that service to your health plan for payment or healthcare operations purposes. We will honor that request unless disclosure is required by law.
Get a List of Certain Disclosures
You may request an accounting of certain disclosures of your health information made during the six years before the date of your request.
The accounting will not include certain disclosures, including many disclosures made for treatment, payment, or healthcare operations.
We will provide one accounting during a 12-month period without charge. We may charge a reasonable, cost-based fee for additional requests during the same 12-month period.
Get a Copy of This Notice
You may request a paper copy of this Notice of Privacy Practices at any time, even if you previously agreed to receive it electronically.
Choose Someone to Act for You
If you have given someone medical power of attorney or another person has legal authority to act on your behalf, that person may exercise your rights and make choices about your health information.
We will verify that the person has appropriate authority before taking action.
File a Complaint if You Believe Your Rights Have Been Violated
You may file a complaint with LIVE Clinic if you believe your privacy rights have been violated.
You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights.
We will not retaliate against you for filing a complaint.
Your Choices
For certain health information, you may tell us your preferences about what we share.
In certain situations, you may tell us whether we may:
Share information with your family, close friends, or others involved in your care
Share information in a disaster-relief situation
If you are unable to tell us your preference—for example, if you are unconscious—we may share information when we believe doing so is in your best interest and is permitted by law.
We may also share information when necessary to reduce a serious and imminent threat to health or safety.
Marketing and Sale of Information
For certain marketing activities, we will obtain your written authorization when required by law.
We will obtain your written authorization before selling your protected health information when required by law.
We do not sell patient health information.
How We Typically Use or Share Your Health Information
Treatment
We may use your health information and share it with other healthcare professionals involved in your treatment.
Example: We may share relevant information with a specialist, laboratory, hospital, pharmacy, or other healthcare provider involved in your care.
Healthcare Operations
We may use and share your health information to operate our practice, improve the quality of your care, and manage our services.
Example: We may use health information to evaluate the quality of care provided by our clinicians or improve clinical operations.
Payment
We may use and share your health information to bill for services and obtain payment from health plans or other responsible parties.
Example: We may provide information about a visit or procedure to your health insurance company so that it can process a claim.
Other Ways We May Use or Share Your Information
We may use or disclose your health information in other circumstances when permitted or required by law.

We may disclose health information for certain public-health and safety activities, including:
Preventing or controlling disease
Reporting certain communicable diseases when required by law
Reporting suspected abuse or neglect when required by law
Reporting adverse reactions or product-safety concerns
Preventing or reducing a serious threat to someone’s health or safety
Complying With the Law
We may disclose information when federal or state law requires us to do so.
Health Oversight Activities
We may disclose information to health oversight agencies for activities authorized by law, such as audits, investigations, inspections, disciplinary proceedings, or licensure matters.
Workers’ Compensation
We may disclose health information as authorized by and to the extent necessary to comply with workers’ compensation laws and similar programs.
Law Enforcement and Government Requests
We may disclose health information to law-enforcement officials or government agencies when permitted or required by law.
Medical Examiners and Funeral Directors
We may disclose health information to coroners, medical examiners, or funeral directors when permitted by law.
Organ and Tissue Donation
When applicable, we may disclose health information to organizations involved in organ, eye, or tissue donation and transplantation as permitted by law.
Lawsuits and Legal Proceedings
We may disclose health information in response to a valid court or administrative order, subpoena, discovery request, or other lawful process when applicable legal requirements have been satisfied.
Research
We may use or disclose health information for research when permitted by applicable law and when required protections are in place.
Substance Use Disorder Records
To the extent that LIVE Clinic receives or maintains substance use disorder patient records protected by 42 CFR Part 2, additional federal confidentiality protections may apply.
Such records will not be used or disclosed in civil, criminal, administrative, or legislative proceedings against a patient based on those records unless the disclosure is permitted under applicable federal law, including when appropriate patient consent or a qualifying court order and subpoena are obtained.
Reproductive Healthcare Privacy
We will use and disclose protected health information related to reproductive healthcare only as permitted by applicable law.
When federal law requires an attestation before certain disclosures of reproductive healthcare information, we will obtain the required attestation before making the disclosure.
Our Responsibilities
We are required by law to maintain the privacy and security of your protected health information.
We will notify you as required by law if a breach occurs that may have compromised the privacy or security of your protected health information.
We must follow the duties and privacy practices described in the Notice of Privacy Practices currently in effect.
We will not use or disclose your health information other than as described in this notice unless you authorize us to do so in writing or another use or disclosure is permitted or required by law.
If you give us written authorization, you generally may revoke that authorization in writing at any time. Your revocation will not affect actions we already took in reliance on your authorization.
Electronic Communications
LIVE Clinic may communicate with patients electronically when appropriate and permitted by law.
Patients should understand that ordinary email, text messaging, website forms, social-media messages, and similar communication methods may have privacy and security limitations.
Please do not use ordinary electronic communications for medical emergencies.
Changes to This Notice
We may change the terms of this Notice of Privacy Practices.
Changes may apply to all protected health information we maintain, including information created or received before the change.
When this notice is materially revised, the current version will be available upon request, at our office, and on our website.
